ABSTRACT

Objective To explore the expression of heme oxygenase-1 (HO-1) in gastric mucosa of patient with portal hypertensive gastropathy (PHG),and its role in the pathogenesis of PHG.Methods The specimens were obtained from the gastric mucosa of 22 healthy subjects,20 portal hypertension (PHT) without PHG,and 22 PHG patients.Histological changes of the gastric mucosa were detected,and portal venous flow (PVF) was measured.The expression of HO-1 protein in gastric mucosal specimens was assessed by immunohistochemistry and Western blot analysis,respectively.The relationship between HO-1 expression and PHG severity score,HO-1 expression and PVF,PHG severity score and clinical parameters were investigated.Results HO-1 protein expression in gastric mucosa of PHG and PHT was significantly higher than that of the control (P < 0.05),while there was no significant difference in this variable between PHG and PHT (P > 0.05).The positive correlation was detected between HO-1 expression and PHG severity score in PHG patients (r =0.459,P <0.05),however,PHG severity score was irrelevant to severity of esophageal varice (r =0.059,P > 0.05) or to Child-Pugh classification (r =-0.001,P > 0.05).Of all the patients with PHT and PHG,no significant correlation was found between HO-1 expression and PVF (r =0.071,P > 0.05).Conclusion HO-1 protein is up-regulated in gastric mucosa of PHG patients,which may contribute to gastric circulation disorder of PHG.

ABSTRACT

ObjectiveTo investigate the popularity of the sedated gastrointestinal endoscopy in mainland China.MethodsThe survey on the popularity of the sedated gastrointestinal endoscopy was conducted with questionnaires sent by e-mails or telephones to the GI endoscopy units of three levels of hospitals.ResultsFrom May,2010 to November,2010,169 units from 28 provinces (90.3% ) were enrolled,including 46 hospitals of grade Ⅲ,91 of grade Ⅱ and 32 of grade Ⅰ.Sedated gastrointestinal endoscopy were performed in 110 (65.1%,110/169) hospitals,i.e.93.5% (43/46) grade Ⅲ hospitals,68.1%(62/91) grade Ⅱ hospitals and 15.6% (5/32) grade Ⅰ hospitals.Significant difference was observed between the grade Ⅲ and the grade Ⅱ hospitals (P < 0.05 ),so was between the grade Ⅱ and the grade Ⅰ hospitals ( P < 0.05 ).Propofol was frequently used in sedation in 98 hospitals ( 89.1% ).The only 9.1%( 10/110)hospitals,which had full-time anesthetists or anesthetic nurses,were all grade Ⅲ hospitals.ConclusionThere has been great progress in the popularity of sedated gastrointestinal endoscopy in mainland China during the past decade.However,it is less frequently pefformed in grade Ⅰ and Ⅱ hospitals.Shortage of full-time anesthetists is still a problem.

ABSTRACT

Objective To investigate the current status of conventionai diagnostic/therapeutic gastrointestinal (GI) endoscopy (conventional gastroscopy/colonoscopy and endoscopic polypectomy,et al.) in mainland China.Methods The survey was conducted by a questionnaire sent via e-mail or telephone to the hospital-based GI endoscopy units,including three levels of hospitals (Third-Grade Hospital Classification in China).Results From May 2010 to November 2010,169/279 (60.6%)units were enrolled,which covered 28 provinces (90.3%) in mainland China.Among the 169hospitals,147(87.0% ) hospitals performed GI endoscopy,and all the hospitals performed gastroscopy (100%).Furthermore,100% of the tertiary hospitals,and 93.9% (138/147) of the second-level hospitals performed colonoscopy,significantly higher than that of the first-level hospitals (25.0%)(x2 =60.9,P<0.01).All the tertiary hospitals (100%),and 79.8% (71/89) of the second-level hospitals perform endoscopic polypectomy,significantly higher than that of the first-level hospitals (16.7%,x2 =20.0,P<0.01,P<0.05).Among the 147 hospitals with the ability to perform GI endoscopy,74 hospitals (50.3%) performed endoscopic retrograde cholangiopancreatography (ERCP).Furthermore,76.1% (35/46) of the tertiary hospitals performed ERCP,significantly higher than that of the second-level hospitals (43.8 %,39/89) (x2 =12.7,P<0.01),and none of the 12 first-level hospitals performed ERCP,endoscopic ultrosonography (EUS) or EVL/EVS(endoscopic oesophageal varices ligation/sclerotherapy).Conclusions The conventional diagnostic GI endoscopy (routine gastroscopy and colonoscopy) was popularized in the tertiary and the second-level hospitals,and there was still much to improve for the first-level hospitals.With regard to the conventional therapeutic endoscopics,ERCP and EVL/EVS were more popular in the tertiary hospitals,while the second-level hospitals have much to improve.

ABSTRACT

Objective To explore the clinical features and possible carcinogenesis mechanism of the ulcerative colitis (UC)-associated colorectal cancer. Methods From 1984 to 2008, 6 clinical cases of UC-associated colorectal cancer were collected at Peking Union Medical College Hospital. The characters of morbidity, clinical features, pathology type, treatment and prognosis were analyzed.Immunohistochemistry(IHC) was performed to study the protein expression of adenomatous polyposis coli protein(APC), β-catenin, P53 and Wnt-1 in the specimens. Results The canceration of UC was 1.1 %, higher in female cases (5/6), the average duration was 14.3 years. All cases presented typical UC manifestation, most involved total colon (5/6) and none of them complicated with primary sclerosing cholangitis (PSC). There were 4 rectal cancers and 2 descending colon caners the UC cases collected. The major pathology type was adenocarcinoma with poor prognosis. The positive protein expression ratio of APC, β-catenin, P53 and Wnt-1 were 6/6, 6/6, 5/6 and 6/6 respectively.Conclusion The rectal cancer should be monitored and prevented in UC patients with total colon involved and long disease duration. Multi-pathway may possibly be involved in the carcinogenesis.

ABSTRACT

Objective To explore the role of extracellular signal-regulated kinase(ERK)in the apoptosis of hepatic stellate cells(HSC) induced by Arg-Gly-Asp-Ser (RGDS) tetrapeptide. Methods The cultrued HSC cells were divided into six groups including untreated control, fibronertin (FN), FN+RGDS (25 mg/L), FN+RGDS (50 mg/L), FN+RGDS(100 mg/L) and FN+Arg-Gly-Glu-Ser(RGES, 100 mg/L) groups. 3H-thymidine incorporation, transmission electron microscopy and TUNEL were employed to estimate the influence of RGDS on proliferation and apoptosis of HSC. The HSC adhesion rates were observed by toluidine blue colorimetric assay. The expressions of ERK mRNA and ERK protein in HSC were detected by RT-PCR and Western blotting. Results Compared with control and fibronectin groups, RGDS tetrapeptide at concentrations of 25 mg/L, 50 mg/L and 100 mg/L inhibited the proliferation of HSC(P
